Test requisition forms
A bulk supply of Genetic and Neoplastic request forms with your information pre-printed can be ordered by calling the Clinical Orders Department at 800-862-1088 or 608-262-0402. If you use form downloaded from the website, you must include your billing account and facility address in upper right hand box.
Specimen Shipper Kits Available!
Specimen shipper kits can be used to send human blood and other tissues specimens the WSLH Cytogenetics and Molecular Genetics Laboratories. The WSLH provides these kits free of charge to our customers. The kits include all the necessary items to ship human clinical specimens, kit instructions for packing up the specimen, and a pre-paid return mailer envelope.
We currently offer specimen shipper kits for sending blood, buccal, blot spot cards, and saliva samples.
You can request a shipper kit the following ways:
- Call the laboratory at 608-262-0402
- Email the laboratory at cytogenetics@mail.slh.wisc.edu
- Information needed: name an address of person to send kit to (cannot send to PO boxes), number of kits requested, and contact information (phone and/or email).
